We try to streamline your visit by making the important health questionnaire available on our website, allowing you to answer in the privacy of your home when you have access to your health history and names of your specialists or your previous OB-GYN.
If you are not able to fill out the health questionnaire online, please bring a list of all the medications you are currently taking as well as any previous medical records you may want to provide.
